Physically qualified as prescribed by the U. S. Department of Transportation; Valid D. O. T. medical card; See FMCSR Part 391. 41 for specific physical, mental, and vision requirements. Valid class A Commercial Driver Instruction Permit (CDIP) with air brakes or CDL Class A with air brakes. Copy of motor vehicle record (MVR) for the past three years. Acceptance into the program may be contingent on the quality of the driving record. Contact admissions advisor if MVR has major or serious violations. Pre-enrollment drug screening required: The required drug/alcohol
